Make a Claim - CTP Insurance NSW

Call us on 1800 795 277 to make a claim for personal injury. Depending on your circumstances, you will need to submit either an Accident Notification Form or a Personal Injury Claim form. Our Claims Officers can help you lodge the appropriate form.

If you hold GIO CTP Insurance and are involved in an accident and are considered to be at fault, you will need to complete a Notice of Accident Report form if a claim for personal injury is made against you.

Download the appropriate form and post to us	Which form do you need? If you were not at fault in the accident To immediately claim up to $500 for early medical treatment expenses, complete the Accident Notification Form (PDF 495KB). If you wish to make a claim for expenses in excess of $500.00 or you think you will incur losses for more than 6 months, complete the Personal Injury Claim Form (PDF 520KB). If you were at fault in the accident You need to complete a Notice of Accident Form (PDF 175KB). Send the correct form to: CTP Claims, GPO Box 4091, Sydney NSW 2001
Information you need to provide	Your name, address and contact phone numbers Your part in the accident The registration and make of the vehicles involved in the accident Who was driving the vehicle or responsible for it at the time of the accident. If you send us in the form, a diagram of the accident would also be helpful.
What next? Processing your claim	We will provide you with a claim number, and the name of the person handling your claim once we receive your form. For an Accident notification form: You'll be notified within 10 working days of receipt whether you: Can immediately claim up to $500.00 for your treat Need to lodge a Claim Form. For a Claim Form Your claim is investigated to confirm the driver is at fault. During this investigation period, we request that you or your treating practitioner provide advice of all the treatment received. Within 3 months of receipt of the Claim Form you'll be notified whether the claim has been accepted. If accepted, you will receive reimbursement and ongoing support with your rehabilitation. Over time a settlement will be reached.
